Enrollment Forms 2014 -2015
Launch the media gallery 1 playerEnrollment is August 4th from 8:30 am until 3:00 pm. Forms are now available below to print and fill out. This will make it a quick process. Please make sure your child is enrolled on this day. Thank you! See you then! When you fill out enrollment forms note that bus numbers have changed you may add this when you arrive at the school.
1. Friend School Enrollment Form - All Students
2. Authorization for Medical Care Form- All Students
3. Permission to Administer Medication- Students with medication including Tylenol
4. Transfer Student Contract- All transfer students that have not filled out this form.
5. Initial Enrollment Form - For ALL Pre-K and first time in a public school Kindergarten students.
6. Movies - 7th and 8th Grade ONLY
7 Physicals- ALL 7th and 8th Graders / 5th and 6th Grade are optional
8. Cell Phone Release Form- All students with cell phones
9. Student Computer and Internet Usage Forms- Students 1st- 8th Grade
10. Home Language Survey- For Students who speak another language at home other than English Coming Soon
